What is this model of care?

Sweetser’s Certified Community Behavioral Health Clinic (CCBHC) in Sanford offers high quality, integrated, evidence-based substance use and mental health services. CCBHCs are designed to increase timely access to mental health and substance use services, including MAT (Medication-Assisted Treatment) for substance use disorders. The clinics provide a comprehensive array of services needed to create access, stabilize people in crisis with mobile crisis services, and provide the necessary treatment for those with the most serious, complex mental illnesses and substance use disorders.

Integrated Outpatient Mental Health and Substance Use Services

  • Short-term & long-term therapies, medication management and MAT
  • Evidence-based treatments for Co-Occurring or Substance Use or Mental Health disorders that utilize an individual or group therapy approach to support symptom reduction

Targeted Case Management & Care Coordination

Peer & Family/Caregiver Supports

Collaborative care for Uniformed Service Members & Veterans

  • Partnerships with the nearest Department of Veterans Affairs’ medical center, independent clinic, drop-in center, or other facility of the Department

How do I access services?

We develop an individualized treatment plan based on information obtained through the comprehensive evaluation, and the goals and preferences of the person receiving services. The plan will address the person’s prevention, medical, and behavioral health needs.

The clinic provides services to anyone seeking help for a mental health or substance use condition, regardless of their diagnosis, place of residence, or ability to pay. We offer a financial assessment, which determines sliding scale fees and payment plans as needed.
